BLEEDING IN THE LATE WEEK OF PREGNANCY
Some pregnant women bleed after about 28th week of the pregnancy. This is as a result of bleeding from the genital tract after the 28th week of pregnancy and before the end of second stage of labour, which is known as ante partum hemorrhage.
Apart from cervical causes listed in causes of bleeding in early stage of pregnancy and bleeding due to SHOW at the onset of labour, there are only two clinically recognizable causes. These are; PLACENTA PRAEVIA and ACCIDENTAL HAEMORRHAGE
PLACENTA PRAEVIA:- This is a condition where the egg-cell embeds near the isthmus, where there is more fibrous tissue, little glycogen and few blood-vessels. With elongation of isthmus there is risk of bleeding, either abortion occur in the first three months or, if the pregnancy continues there will be unavoidable Hemorrhage from the site of low-lying placenta praevia, and accidentally from placenta itself. Placenta Praevia occurs in about 10% of pregnancy.
There are different kinds of placenta praevia. First Degree, Second Degree, Third degree and fourth degree.
FIRST DEGREE: Is a situation where most of the placenta lies in the upper segment and only the lower pole encroaches upon the lower segment.
SECOND DEGREE: The edge of the placenta reaches the internal OS of the cervix. This used to be called a marginal placenta praevia.
THIRD DEGREE: The placenta covers the undilated cervix; this corresponds with a partial or incomplete placenta praevia.
FOURTH DEGREE: Here the placenta covers the OS of fully dilated cervix.
With placenta praevia, bleeding may occur before, during and after the birth. Always complain to your health advisor in any case.
ACCIDENTAL HAEMORRHAGE: This is a bleeding from a normally situated site from the 28th week of pregnancy until the birth of the baby. In conceal accidental hemorrhage (7%) there is no vagina bleeding, and all the blood lost is retained inside the uterine cavity. This is the most serious variety. In revealed hemorrhage (42%) all of the blood lost pass through vagina and this is the mildest type of all. Thirdly there is mixed Accidental Hemorrhage (50%), where although there is some vagina bleeding, most of the blood is retained. This is an insidious and dangerous condition.
Symptoms of accidental hemorrhage: Headache, Sickness, Feet Swelling and Visual Disturbances May Occur, Recurrent Vagina Bleeding, Abdominal Pain, Pain And Tender, Faintness.
